Please take a moment to update your information BEFORE your visit if you have had any changes to the following:

• Update Demographics (phone, address, etc)

• Update Insurance/Payment Information

If you have not had an appointment since the start of the current dance year, please also update the Informed Consent/HIPAA paperwork.
